• While I do not accept direct insurance payments, I can supply an insurance compliant receipt with all service and diagnosis codes required by insurance carriers. Check with your insurance provider to see if your plan allows you to choose a provider of your choice or if your benefits are restricted to the plan’s provider. Many Blue Cross and United Health Plans have a $150 eyewear benefit which you can use at the supplier of your choice by submitting a receipt for reimbursement.
